Background:

Many institutions request/require a report of the health status of animals prior to receiving them. In order to protect the health and welfare of animals shipped from UIUC and animals at other institutions, the receiving institution’s agreement to accept the animals should be verified before shipment.

Policy:

The receiving institution’s agreement to accept animals from UIUC is required before animals are shipped. A report of the health status of animals proposed for shipment will be sent from the Division of Animal Resources to the laboratory animal veterinary office of the receiving institution. Notification of acceptance from the receiving institution’s laboratory animal veterinary office to the DAR will be received before animals are shipped.

Implementation:

  1. Role of Investigator    The UIUC investigator will provide the DAR with information required to produce a health status report and contact information for the receiving institution’s laboratory animal veterinary office and the receiving investigator. Animal Export Request Forms specifying required information will be available through the DAR and the unit supervisor. The investigator will hold shipment of animals pending notification from DAR that the receiving institution has agreed to accept the shipment. The form is available at: http://www.dar.uiuc.edu/forms/animalexportform.htm.

  2. Role of Animal Care Unit    The unit supervisor will provide the Animal Export Request Form to the investigator, and will work with DAR to produce an accurate and current health status report. The unit supervisor will work with the investigator to arrange shipment, and will ship animals after notified to do so by DAR.

  3. Role of DAR    The DAR will provide Animal Export Request Forms to investigators and supervisors as requested. When provided with a completed Form, the DAR will produce, fax, and mail a health status report within 5 business days. The DAR will notify the investigator and/or the unit supervisor when the receiving institution’s laboratory animal veterinary office agrees to accept the shipment.

  4. Role of IACUC    The IACUC will oversee compliance with this UIUC policy
